Current Foster Parents
The following forms are used by current foster parents to support the ongoing care, documentation, and medical needs of children placed through People Places.
- Annual Medical Evaluation (PDF)
Required annual physical for children in care. - Dental Form (PDF)
Documentation of routine or follow-up dental visits for children in care. - Medical Follow-Up / Treatment Form (PDF)
Used after medical appointments or recommended treatments for children in care. - Tuberculosis Screening Form (PDF)

Health clearance for adults providing care. - Allowance Chart for School-Age Children (PDF)
