Prospective Foster Parents

Considering becoming a foster parent? These guides provide an overview of the Foster Parent role, home requirements, and what to expect as you begin the process with People Places.

 Please note: At People Places, foster parents are referred to as Teaching Parents. You will see this terminology used throughout the paperwork.

Current Foster Parents

The following forms are used by current foster parents to support the ongoing care, documentation, and medical needs of children placed through People Places.

• Annual Medical Evaluation (PDF)
  Required annual physical for children in care.
• Dental Form (PDF)
  Documentation of routine or follow-up dental visits for children in care.
• Medical Follow-Up / Treatment Form (PDF)
  Used after medical appointments or recommended treatments for children in care.
• Tuberculosis Screening Form (PDF)
  Required screening for adults in the home.
• Medical Report for Adult Caretakers in the Home (PDF)
  Health clearance for adults providing care.
• Allowance Chart for School-Age Children (PDF)
  Guidelines for age-based youth allowances.
• Consent for Psychotropic Medication (PDF)
  Authorization form required prior to the prescription or administration of psychotropic medications for children in care.
